What a community need assessment should answer

Assessments become useful when they end in decisions — where to start, who to serve first, and what to stop planning.

Community need assessments frequently produce a long descriptive report and no decision. The more useful version is shorter and answers a specific set of questions that a partnership can act on within a quarter.

Four questions worth answering

  1. Which conditions and barriers appear most often in this community, according to both public data and the people living there?
  2. Which of those barriers can a non-clinical community program realistically affect?
  3. Who is currently least likely to be reached by existing programs?
  4. What would count as evidence, twelve months from now, that access improved?

Use public data, then ask

Publicly available county and state health data is a starting point for identifying patterns. It rarely explains why a program is not reaching someone. That explanation comes from conversations with residents and frontline staff — transportation windows, language access, documentation requirements, hours of operation, and prior experiences that shaped trust.

Equity work becomes concrete when a program names who it is currently missing and changes something specific to reach them.

Keep the data footprint small

Assessment does not require collecting sensitive individual-level information. Amana Healthcare's practice is to rely on aggregate public data, structured community conversations, and partner-reported operational counts, with clear limits on what is collected and how long it is kept.
