Closing the loop on community referrals

When a clinic, school, or community organization refers someone to a food or wellness program, the referral usually leaves the building and is never heard from again. Nobody intends this. It happens because 'closing the loop' is rarely defined, and because no single person owns the step between the referral and the service.

It is worth being precise about scope. Amana Healthcare's care coordination work is non-clinical: helping people understand what a program offers, what documents they need, where to go, and who to ask. Clinical decisions, treatment, and licensed services remain entirely with the participant's own providers.

Write the definition of 'closed'

A referral is not closed when it is sent. Partners should agree, in writing, on which of the following counts as closure — and then measure it consistently.

1. Referral received and logged by the receiving organization.
2. Participant successfully contacted, with a defined number of attempts.
3. Eligibility and next steps explained in the participant's preferred language.
4. Participant enrolled or connected to the service.
5. Outcome reported back to the referring partner.

Referral volume follows confirmation. Partners keep sending people when they can see what happened.

Respect data boundaries from the start

Closing the loop requires sharing something back — but only the minimum needed. Amana Healthcare's practice is to agree on the smallest useful data set, document how it is stored and for how long, and keep individual-level information out of partner reporting entirely. Aggregate counts are almost always sufficient for program management.
